Kiri and Iwai have made plans to cut her hair during the school trip. Iwai is enjoying the new experiences she is having as Kiri frets over his relationship status with Iwai. The school trip is thrown into chaos when a new Author makes an attempt on the Queen.
Iwai carries the wounded Emily as a forest fire spreads. She sympathizes with Emily telling how much their father cared about them. The murderous Kiri appears and walks up to Iwai touching her body and then violently cuts her hair. Kiri then regains conscious and passes out. Iwai realize she mustn’t forget how painful fear is, and her hair suddenly grows long again. Later, as Kiri is being hospitalize at a clinic owned by the Gossip after Emily called for a rescue department. Iwai and Emily take a bath together as they recover from their injuries and form a sister relationship. Emily intimates Iwai if she and Kiri are dating or ever had sex, Iwai admits she has feelings for Kiri, in spite of how scaring he can be. Meanwhile, Kiri wakes up from his medicinal bed he gets an unexpected visit by Lady Violet Witchy. She gives Kiri a rough discussion on the Killing Goods and their Authors. Then she impulsively gives him his first kiss before saying that his days as a murder are far from done. The next day, Kiri is overjoying to cutting Iwai's hair once again with his scissor.
Mr. Tsuyuki and Mr. and Mrs. Hasue, among others, have formed a mob. Mr. Tsuyuki convinces the others that they must kill Takuma. Hotaru looks for her brother, who is nowhere to be found, but she finds the book about Hell Girl, and runs away with Takuma.
Kazuki, Tokiko, and Bravo infiltrate the LXE's base only to find out Dr. Butterfly and his army of Homunculi are targeting Kazuki's school. Baba Shouko is a teacher everyone dislikes because she frequently scolds students and is very strict. A fake Hotline to Hell was created by somebody and it is circulated among students who were just scolded by Baba sensei. Tracing was carried out and they concluded that a student named Ikami Waka created and circulated the fake Hotline to Hell. However, Ikami claims that she did not do it. On the same night, another teacher named Kuriyama Mami calls Ikami and tells her that it was Baba who made the fake Hotline to Hell, blames her for it and told Ikami to come to school. Mami-sensei waits in school for Ikami to come and tried to convince Ikami to access the real Hotline to Hell to send Baba to hell. To her surprise, the one who turned up is Ai. Mami and Ai already knew each other from their pasts. The truth is Mami was the one who created the fake Hotline to Hell and blamed the students for it. It was all because Baba scolded Mami when she was young and she blames Baba for all her problems. Baba comes and types in Mami's name into the real Hotline to Hell, as a punishment for an attempt to use her students as sacrifices. Baba agrees to go to hell as she blamed herself for coaching a teacher like Mami.
Yuzuki lived happily with her parents, until her father, a bus driver, died in a bus crash due to faulty brakes. The whole town accused the Mikage family of murder and being irresponsible. Sick and without means to support herself she decides to leave.
A young woman is trying to get her ex-boyfriend to get back together with her for the sake of her and the child she's carrying. She has the straw doll in case he refuses, but she is obsessively pursuing a reunion. Her story is intersected with Hone Onna's story. Originally, Hone Onna was a young woman named Tsuyu who was sold into prostitution to cover the debts of her lover. Tsuyu used to make paper ball balloons to sell. After a few years, Tsuyu is joined by a younger prostitute named Kion. Kion feels inferior to Tsuyu and ultimately betrays her, resulting in the violent death that transforms Tsuyu into Hone Onna. Back in the present day, the young lady sends her ex to hell, then attempts to jump off a bridge. Hone Onna realizes the spirit of Kion, who was wounded by her lover after she revealed her pregnancy and then jumped off a bridge, has been possessing women and forcing them to kill themselves by jumping off bridges. Kion tells Hone Onna it was nice to see her again, then vanishes, still on her path of vengeance. Hone Onna asked Ai to please pick Kion up someday, as Ai saved her from an eternity of sorrow and anger.
Tokiko proceeds to eliminate the twins but is stopped by Kazuki who argues that they are still human and deserve to live. Shusui inflicts a critical wound on Kazuki while he is distracted. However, Ouka uses her buso renkin and absorbs Kazuki's wound damage claiming that Kazuki is someone they can trust. As Ouka begins to die, Kazuki, Tokiko, and Shusui are able to avert her death with the healing properties of the kakuganes. The next day, Kazuki and Tokiko visit Ouka in the hospital who reveals that her brother had left to train in order to rid himself of LXE's corruption.
Tokiko attacks Papillion in an attempt to settle the debt Kazuki made with him. Papillion makes no move until she threatens the large flask behind, which contains something he wishes not to be destroyed. Tokiko, thinking its his weapon to destroy the world, manages to crack the flask open only to reveal a white kakugane, made in one month from scratch. As everyone watches, the moon shines a light like the sun, proving that Muto Kazuki still lives on. Using aid from the General and others, Buster Baron launches into space to retrieve Kazuki. But upon his return, Papillion chooses this time for his duel. What outcome will this duel between Papillion and the now human Kazuki be? What will happen to Victor? Yayoi's sister, Sumire is missing. She is constantly hearing the sound of bubbles from underwater. She after a while begins to realize her sister is dead. But because she doesn't know the identity of the killer, she can't get onto the website to get her sister's revenge. She blames herself for Sumire's death because Sumire vanished on a day Yayoi didn't walk with her. Yayoi falls into a fountain and has a vision of what happened to her sister. Sumire was kidnapped, raped, killed, and dumped in a lake, which explains the bubbles. Yayoi knows the identity of the killer and sends him to Hell for Sumire.
After a short battle, Victor concludes that it would take too much time to defeat Kazuki and leaves the school. After reuniting with Captain Bravo, Papillon leaves to research Victor in order to turn Kazuki back into a human being. The Alchemist Warriors find and engage Victor in battle. Meanwhile, Kazuki, Tokiko, Gouta, and Papillon arrive at the school and meet a mysterious figure who introduces herself as Victoria and leads them to her brain in a vat mother, Alexandria, the creator of the black kakugane. Alexandria reveals her creation, a white kakugane, and explains it is able to negate the effects of the black kakugane. A washed up writer decides to take revenge on a producer friend of his who became famous. During his attempted revenge the producer's wife is killed and their son is framed. The son contacts Hell Girl but decides not to go through with it. Shortly after this the producer is killed by the writer, but before the writer can kill the son a woman whose child he killed sends him to hell. This show is significant because it introduces character Takuma Kurebayashi, whom Ai empathizes with.

Honami then finds out that Nene is trying to send her to hell, but then kills herself so that Nene doesn't need to go to Hell too. Also where Ren's past is revealed showing that he was once a sword.
Kamisaka Rokurou is a novelist whose mystery genre book, The Skyscraper's Shadow, has gotten publicity because of strange incidents resembling its plot. The first victim's older sister, Michio Yui, meets with Kamisaka and the murderer, Chouchi Hiroto, to see which one of them is to blame. She intends to send one of them to Hell.
Rika finally makes contact with Hanyu, who tells her that for her to return to her old world, she must find and dispose of a piece from her old world.
Kazuki runs into Captain Bravo, engages him in battle but is soon overwhelmed. Despite the information Tokiko is able to relay, Bravo tells him that after he kills Kazuki, he will kill himself to repent for his death. Kazuki, enraged that his failure will result in Bravo's death, pierces Bravo's buso renkin and wounds him. Bravo accepts that Kazuki has become strong and allows him to continue his search for a cure. However, they are then interrupted by Hiwatari's appearance.
Hiwatari engages the four in battle and Bravo is critically wounded in the process. Just before Kazuki and Hiwatari finish each other off, the Alchemist General, Shosei Sakaguchi, stops them explaining Kazuki's assassination has been suspended and that the top priority for the Alchemists is the battle against Victor. A marriage ceremony is preformed. A sweet potato vendor comes around, and the girls fight over the last one. The GJ Club discuss what they will do for the culture festival. Megumi, Mao, and Shion decide to dress up to tease Kyoya. Mori, the Amatsuka family maid, brings something Megumi forgot. Kirara and Kyoya dance, and Kyōya says something that embarrasses her.
Kazuki and Tokiko befriend the Hayasaka twins, Shusui and Ouka. However, the twins are actually humans working with the LXE ordered to kill the Alchemist warriors at the school. During the night, Bravo takes Kazuki out to the woods for special training while Tokiko protects the kakugane acquired from the previous battle. After the battle (Kanzel and Mazenda got away), Lina can't use her powers, so she (by herself) decides to see the doctor (at mount Rune) who is famous for curing problems. Lina manages to get Xellos (and Martina as a stowaway) up there as well. When they get there, they find out that is doctor is a 10 year old kid, who is actually a relative's real doctor who died long ago. When mixing up a sensitive potion together, Martina manages to slip something in, in hopes of killing Lina only to have the potion blow up and house and everyone in it. In the midst of the debris, they are attacked by monsters. Lina notices that Xellos is using some magical orbs/amplifers to give a power up on his magic. Lina manages to convince Xellos to give her the ampliers, and she puts it one. After killing the monsters, Martina stumbles out of some debris, only to have something ontop her head... The Claire Bible Manuscript!
Yuzuki is still disturbed by her encounter with Ai. Meanwhile, a middle school girl has developed a crush on a middle-aged shopkeeper -- and a grudge against her abusive husband.
After searching hard for the Papillion Mask, they finally found him–although he was definitely not what they were expecting. He supposedly wanted to bargain with Kazuki: the antidote to the Homunculus attached to Tokiko’s body for his Busou Renkin, but Kazuki refused. With a single Kazuki punch, The Papillon Mask was knocked out and brought to be questioned. (The “antidote” was a fake.) Tokiko manages to find Kazuki's body in the ocean and is able to revive him. Meanwhile, Gouta learns that a team led out by Alchemist Warrior Chief Sekima Hiwatari is sent to confirm Kazuki's death. Before Dr. Butterfly dies, he explains that a man named Victor is inside the flask and that he is a being far superior to humans and Homunculi. Kazuki destroys the flask and engages Victor in battle and is seemingly killed after having his kakugane destroyed. Captain Bravo, a member of the Alchemist Warriors and the one who delivered the cure to Tokiko, offers Kazuki to join their mission on combating against the LXE, a group consisting of Human-type Homunculi based in Kazuki's town. Koushaku, having been resurrected by the LXE's leader who is also his great-great grandfather, Dr. Butterfly, and now calling himself Papillon, seeks Kazuki and declares war on him. Yamawaro and Kikuri approach a large mansion belonging to a client but are unable to enter due to an encircling barrier. At night, the owner of that mansion accesses the Hell Correspondence website and types Tsugumi's name. Each sensing the request, Yuzuki, Ai, and Tsugumi make their ways towards the mansion. Once they gather, he explains that he yearned to meet Ai.
Kazuki finds Koushaku in his great-great grandfather's laboratory and attempts to destroy the Homunculus matter. However, due to the interference of Koushaku's younger brother, Koushaku is able to have the Homunculus matter attach to him and is turned into a Homunculus. He then begins to eat everyone in the vicinity until he is killed by Kazuki, who passes out from exhaustion before he is able to deliver the cure to Tokiko.
