His wife arrives at the ER; incapable of believing her husband would be capable of such a violent act. The day begins slowly as Weaver calls to say she'll be late, and Chen thinks she's developing a fever. Then multiple MVAs come in, quickly followed by two children suffering what appears to be smallpox. The staff panics, but not as bad as the patients, when a lockdown is ordered for the ER. The feverish Chen is quarantined with homeless Stan. Lewis takes charge as Weaver arrives and the police won't let her in. The girl infected by smallpox dies and Pratt embarasses Carter in front of her parents. Carter notices Pratt is sick as well, and sends him to Chen's room. Jerry phones the news, and the patients riot, nearly using a gurney to break open the emergency bay doors. Control of the ER is relieved from Weaver by an infection specialist. The staff prepares to stay, still not having a confirmation on whether it is smallpox or not, and the lockdown continuing as a result. Carter and Abby kiss after being quarantined together.

While all of this has been going on, Elizabeth has been in London, working at her father's hospital, where her supervisor doesn't like the American mannerisms and vocabulary she's picked up. She ultimately decides to return to Chicago.

Carter quits and walks out of the hospital. Benton confronts him with a reminder of what happened to Carter's cousin Chase. Carter hits him but then breaks down in his arms. Later, as Weaver waits for word that Carter has gotten on his way to Atlanta, we see Benton sitting silently next to him on the plane.

Lewis, Neela, and Ray treat a police officer shot during a robbery. His 15-year-old son must make a difficult decision on his father's course of treatment and he turns to Ray for advice. Ray promises to help the boy explain his decision to his mother, but he instead leaves for a gig and later gets an angry earful from Neela after the boy is browbeaten into changing his mind. Abby and Dubenko help a young girl whose mother does not want her to have critical surgery at County, but her condition worsens before her transfer can go through. Luka begins to take a more active role in Alex's life after he's suspended for bringing a copy of Maxim to school. An exhausted Chen returns to work, where Pratt finds her stealing potassium.
